Hospice Savannah uses a team approach to your care. Your individual care plan will determine your team members. You and your family are a vital part of this team. Other members may include:

Nursing Services
Our nurses are the team leaders; they will develop and manage a plan of care for each patient and family. The hospice nurse will assess the patient and with the support of your physician and our Medical Directors, make every effort to ensure that your symptoms are controlled and well managed. Your nurse is prepared to address any questions you may have about what to expect throughout the illness.
Social Workers
Our social worker's role is to assist the patient and family with the emotional aspects of end-of-life care. They also will also be able to provide information and referral to resources available in your area.
Chaplains
Our chaplains are available to support patients and families with the spiritual aspects of end-of-life care. Their role is to supplement, not to take the place of, the family’s minister or religious leader.
Certified Nursing Assistants
Our CNA’s provide a variety of personal care services to meet the needs of our patients.
Volunteers – The Heart of Hospice Savannah!
Our trained volunteers are available to assist you and your family in various ways. Some areas in which a volunteer may prove to be helpful include: companionship for the patient or family members; sitting with the patient to provide relief for the caregiver for short periods of time; running errands to the post office and grocery store, reading to the patient; writing letters; tidying the patient area; helping with transportation; assisting with meal preparation; provide pet companionship; or simply being there. We encourage you to accept the support of our dedicated volunteers. They generously give of themselves because they want to help you. Storykeeping and music volunteers are available in homecare and nursing homes. Please tell your nurse, social worker, chaplain, or CNA if you would like to have a volunteer visit you. |
