Our model of care ensures maximum benefit for patients who have been diagnosed with a serious, progressive illness like congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney disease, cancer or dementia by using best practice standards for care.
The Steward Center for Palliative Care adheres to national quality standards and implements strategies to follow patients throughout the continuum of their care. This evidence-based, multidisciplinary approach integrates your current team of specialists including:
- your primary care physician
- a board-certified palliative care physician
- nurse practitioners
- nurses
- social workers
- chaplains
- dieticians.
This team treats and manages your symptoms within the four corners of wellness: physical, social, psychological and spiritual. A wealth of evidence demonstrates that palliative care improves quality of life and helps those living with a serious illness to live longer, especially when it is a fixture throughout the continuum of a patient’s care.

GUIDE
The GUIDE model offers enhanced services for people living with dementia along with support for their caregivers. Its’ focus is to bring high-quality dementia care for all individuals, including those in underserved communities. GUIDE recognizes the complex challenges families face in caring for a loved one with dementia. This model provides a framework for delivering care with additional resources that were not available until now. To participate in the model, patients must have a dementia diagnosis.
